Possible example is the state of Utah; where an average of people
is 45-56% Mormans. (Registered members of the Latter Day Saints
religion.
Another isthe Vatican. It's a city-state ruled by that Roman
Catholic church.
The worst case example is the Islamic theocratic state of Iran.

Oh, yes. Some of the beginning colonies of the US was founded on
such a premise.

There is a real problem here. To some denominations that does not
believe that God continues to reveal or talk to us, how will we
know that God is acting? In Utah, the Morman fathers claim that
they are God's apostles on earth. They are, in a sense, God's
government here on earth, by their proclaimed authority from God.
It is less than perfect.
Then look at the kingship of David? Is that theocratic?

Well, as I understand it, we (i.e. people) won't be setting up the
theocracy. So we don't have to worry about it. Jesus Christ will set
it up and those living under it won't have had a say in it. :-)

| Will the theocracy set up and ruled by Christ be set up and ruled by
him directly, or will it be done by human intermediaries?
If the latter, how do we recognize that the humans are truly the right
ones, and not some self-proclaimed theocrat?

Christ will rule. The implementation has not yet been divulged. The
effect, if not the implementation will be Christ directly.
There will be no mistake, because only His own will be left and each
will see Christ face to face.
