    One customer aplication (ACMS/DECforms) has a problem with crashing 
    CP-processes.
    
    They always crash with the following exception:
    
    %SYSTEM-F-OPCDEC, opcode reserved to Digital fault at PC=7FF07FA6,
    PSL=03C00000
    
    There is no information in the ATR and SWLUP logs at the time of the
    crash. After the crash you often (not always) see errors like:
    
    - Task canceled in Step EXC_001 in Task RIGTSK0210 in Group RIGGRP02 
      data set hang-up
    - Task canceled due to client or link termination
    - Internal remote request/request interface/DECforms request error
      %ACMS-E-SRVDEAD, The server died unexpectedly
    
    Any ideas about the cause, or how to troubleshoot these problems?

Daan,

You gave us some good info to go on, but we'll need one more piece in
order to track down that PC.  Could you give us the location in memory
where at least the FORMS$MANAGER image is loaded.  The locations of the
other images would help as well...
You get this by going into SDA on your customer's system, and setting
the current process to one of the ACMS CP processes and doing a:

SHOW PROCESS/IMAGE

Could you then paste the output to that as a reply here?

    Daan,
    
    We tracked down the PC you provided.  It's located in a section of code
    that handles some of the DECforms help functions.  Does this exception
    occur when help is in use?
    
    We'd like to know more about the nature of this problem.  How often does
    it occur?  What is the user doing when it happens?  Is there a specific
    set of steps that will cause this exception?
    
    Any details you can provide will help.

    Ruth,
    
    Sorry for not providing the information, but as I mentioned in the
    voicemail, I'm still waiting for information from the customer. The
    interval of the processes crashing is weeks; this interval is not
    regurlaly. 
    It is difficult for the customer to get this information about the user
    actions.
