acms 4.0-0, openvms v6.2, alpha

A customer (Pacificare) has reported a crash of the swlup process.  They
have provided the following information.  They would like to know
why it crashed, and if there is an available fix.

    	According to the OpenVMS V7.1 documentation, the current firmware
    	CD Update release is V3.8 (this is the CDROM version).
    
    	The 8400 should have console V4.1 and PALcode 1.19.
    
    	They should really coordinate this with the hardware people since
    	installing firmware updates can cause other problems to occur,
