I. Introduction
    ************
01. Please supply us with a short summary of your site to help
        us better know you.

II. Administration
    **************

01. Does your site have 3rd party users that will be using DEC field
        test software?

        [ ] No. [ ] Yes.

02. Will DEC Ada T3.4 be used by students? Non-employed students of

SDT Field Test Management has minimum technical support when dealing
with modem issues. We highly recommend that you use a Digital modem,
or one of equal quality and features, in order not to run into modem
problems that cannot be supported and which may effect your timely
        feedback to this test.

02. We depend heavily on sites being able to give us status and
problem reports on a weekly basis. Can you commit to weekly
reporting for the entire duration of this field test?

        [ ] Yes. [ ] No.

03. Once the software portion is shipped, we expect all field test
sites to both install the software and to enter at least one
problem report in 2-3 working days. Will your site do this?

04. If you encounter problems, will you provide us with all sources
and all commands necessary to reproduce the problem?

        [ ] Yes. [ ] No, because
