Has anyone seen anything similar to this?

I was using NOTES with the decwindow interface. I sent mail to a note
author> After the mail was sent, I inadvertently changed something in the
send window so when I went to close it, I was asked if I really wanted to
close it. When I selelcted yes, the windows all disappeared with the
following accvio.
