Jyoti, the missing name means that TomP didn't use the correct (full)
defect number in his submit.  See this line within the submit log:
    ....  Defect number: v32csupportos-224.

should have been  v32csupportos-224-tomp

Oops, I believe this was a cut & paste error based on the subject
of the approval mail I got:
