Mike, you skipped the step of doing a final bci after the bmerge.
The bmerge does a bco as part of its activity, checking out the file,
and locking it. You need to bci anything which was bmerged. It sounds
like that's probably the issue.
You can check to see which files from your set are locked (& writable)
as follows:
bstat -wall
stands for "writeable-all" -- if you're ready to do a bsubmit -all, then
bstat -wall should return _nothing_. (i.e. nothing's locked/writeable).
Note that I've copied odehelp - that's the proper help channel which
includes me as well as others.

Vasu gave me your name as someone I should talk to concerning a bsubmit
problem. I have a set of 381 files. I BCO'ed them, edited them, tested
them, BCI'ed them, BMERGE'd them, tested them again, and finally BSUBMIT'ed
them. For some reason the bsubmit failed on 23 of them. Any idea what
I should do now? Here is a sample for one file.

Great! I'm glad you didn't have any problems in the middle of the
submit. DECwest eng'g has gotten ODE much better than it once was at
trapping things early that will be a problem, which is especially
important in a large transaction such as this one.
Just curious, where are you (in zk3.dec.com? which subnet?), and
about how long did the different phases take (bci -all, bsubmit -all)?

Must have been a particularly unbusy network at the time...

I am in 3W06. I am on the blue net. I did not use '-all' because I have
some unrelated changes that I did not want to check in at this time.
The bci took about 5 minutes for 300 files while the bsubmit took about
20 minutes.
