Using the SOFTWARE.HANDBOOK doctype, the page numbers for the index
seem to be inconsistent.
The first page is numbered "Index-1". The second page is numbered
"2-Index".
Shouldn't the second page (and all other even-numbered pages)
be formatted as "Index-2"?

The placement of the numbers and the text was intentional; it
was treating the string 'Index' more as a running text line than
as a prefix for the page number.
