The following file causes device conversion errors for TERMINAL, LINE, and MAIL.

Per the BL8 release notes, index and contents are not supported for TERMINAL,
LINE_PRINTER, or MAIL.  Index and contents are only supported for LN03 and
POSTSCRIPT.

    BL8 doesn't support automatic inclusion of indexes for certain output
    devices. The test file doesn't contain the <INDEX_FILE> tag. So
    I assumed it would produce a separate index file INDEX3_INDEX.LINE.
