I have recently tried to prepare an sdml file for bookreader purposes.
I use the following command:
$ doc/contents test2.sdml cup.online bookreader
I receive an error once it starts the text formatting:
%RMS-E-FNF, file not found
-TEX-I-FILENAME, 'DOC$ROOT:[TEX.FORMATS]SDMLVOILA.FMT_MACRO;'
%DOC-E-ERROR_FORMATTER, Erros found by the text formatter
Is this file installed using the online_a012 saveset?
Do we have the correct version of VAXdocument? It is currently
V1.2-B.
If the answer is in your savesets, I am not able to access them.

| DOCUMENT is calling an old version of the format file. This usually
occurs because DOC$LOCAL_FORMATS is defined and there is an old online
definition in DOC$LOCAL_FORMATS:DOC$DESTINATIONS.DAT. Remove that
definition and you should be OK. Let me know if you still have
problems.
