With Sheila Lawner's help, I found the answer to my ONLINE
build problem: hanging up in the TEX formatting procedure section
with a memory error:

I inserted <online_chunk> tags every 120 lines in the SDML file that hung
the TEX formatter to fix this problem.
It seems that the ONLINE TEX formatter needs to see <online_chunk> tags
when processing a block of text greater than 300 lines without seeing
any head tags or something. Like when you are presenting large code
examples that go on for pages. These large blocks of text need to be
broken up into smaller chunks to reduce memory impact during the TEX
crunching process--or she will "choke".
