    I have two documents, both of which compile clean for Postscript,
    but both cause DOCUMENT to stack dump on reaching the 
    "%DVC-I-INCLUDING, including input file:" for the contents file,
    when using one of the Bookreader document types.
    
    I am afraid that I need help on this one.
    
    I can provide the relevant files for examination, mail me for details.

    You are using a baselevel that is over a year old. I would suggest
    upgrading to take advantage of the numerous fixes that have occurred
    since then.

    I know that, but I have to use the versions provided.  We work under
    an EASE environment and the DOCUMENT version included is V1.1 with
    no Bookreader, so I consider myself reasonably lucky to have it
    at all !
    
    An upgrade is not an option.   I need a solutions to V1.1 problems.
    
    I managed to somehow fix the problem with the file in .0 don't ask
    me how I did it, I don't know.
    
    I still have an access violation problem with the second file which
    won't go away.
    
    Again, please mail me if the sources are required.

    I don't know what an EASE enviroment is so I really don't understand
    what your restriction is. If you have no Bookreader why do you need to
    build online books? In any case the online tools that you currently have 
    are prototype versions and more than likely the problem you're having has
    been fixed.  I'm sorry but we're not supporting prototype software over a 
    year old that has had many updates since it's release.

    We do have Bookreader on the Workstations.
    
    EASE is a UK/European invention which is sort of a VMS + Tools +
    Utilities + disk configuration kit.    It only contains versions of
    tools and utilities that are proved stable.  The people who actually
    use the tools and utilities have little say in what versions of
    them are specified for the EASE environment.

The solution was the proper use of the <FOOTNOTE> tag. One of our
writers was using the <FOOTREF> tag instead. Neither the tag translator
nor the tex formatter failed with an -E- error but the device
converter bugchecked. The tag formatter should have failed.
