| In all other cases, this problem appears to have been caused by not
completely cleaning up the .DAT files (see also note 277.* in this
conference). Double check that all the changes have been indeed been
made, check doc_v12b_internal_notes for a more complete list of the
changes that need to be made. Do you have doc$personal_formats defined?
If so, check the .DAT files there for old definitions for the online
doctype. If these measures fail, let me know.

I'm having this exact same problem as well. I have gone through
DOC$DESIGNS.DAT and DOC$DESTINATIONS.DAT in DOC$LOCAL_FORMATS and
gotten rid of those lines suggested by the notes. The book still
won't build.
I have noticed something, and I'm not sure that it's a problem
with the kit or a problem with the way it was installed. In
DOC$STANDARD_FORMATS:DOC$DESTINATIONS.DAT, the latest version (in
fact, all versions) refer to SDMLVOILA and not SDMLBOOKR. My
guess is that this might be the problem.
I examined the contents of the kit in CLOSET::KITS_:[DOCUMENT.
V12B.SOFTWARE] (most specifically savesets .B through .E) and could
not find a DOC$DESTINATIONS file of any kind.
I believe the notes say that SDMLVOILA.FMT_MACRO is one of the
files deleted by the installation. If that occurs, and no new
DOC$DESTINATIONS file is moved over, you'd have old destinations
files pointing to SDMLVOILA, when what the build really needs is
a pointer to SDMLBOOKR.
Any ideas or comments?

Mike,
you have anlayzed the installation procedure quite well, however, there
is one major difference. A VAX DOCUMENT installation at a site which had a
previous installation will ask the installer the question, "Do you want to
retain existinig destinations?". If you answer "yes" to this question then the
file DOC$DESTINATIONS.DAT in **DOC$STANDARD_FORMATS** is retained, not the one
in DOC$LOCAL_FORMATS.
I presume you have taken the necessary steps to delete the definition of
obsolete Bookreader logicals, destination defs and design defs from your
*DOC$LOCAL_FORMATS* area. If so, then read on...
The reason you are seeing this error is most probably because at some point you
manually edited the DOC$DESTINATIONS.DAT file in *DOC$STANDARD_FROMATS* and
added the definition for the BOOKREADER destination. The destination dat file
clearly states at the top "DO NOT EDIT THIS FILE". This was not an accident.
Any new installation of DOCUMENT will modify or replace the old destinations
dat file. We clearly intend that you do *not* edit the file manually.
It is a similar incorrect action that caused the error report in reply 12 too.
The way in which you can fix this error is to get the right def for the
Bookreader destination and then place that in your file:
DOC$STANDARD_FORMATS:DOC$DESTINATIONS.DAT
while making sure that the old def is deleted from DOC$STANDARD_FORMATS and
DOC$LOCAL_FORMATS.

I think what I want to know is where to get the vanilla-flavored
DOC$DESTINATIONS.DAT and DOC$DESIGNS.DAT files for placement in
DOC$STANDARD_FORMATS.
I cleared DOC$LOCAL_FORMATS:DOC$DESTINATIONS.DAT out ENTIRELY,
leaving nothing but an empty file. I still have the problem,
mainly because I think we DO have an incorrect DOC$DESTINATIONS
file in DOC$STANDARD_FORMATS.
What I would like to do is get a proper version of the file and
put it in DOC$STANDARD_FORMATS to see if it works. But I didn't
see any in the kit.
Could you point me to one? All versions of ours point to SDMLVOILA
instead of SDMLBOOKR.FMT_MACRO, and I know that is incorrect.
