I apologise if this has already been convered, if it has could someone
point me to the solution.

I got this error with both 1.1 and 1.2 despite going through the effort
of adding all that <LMF> stuff for V1.2 !
Is this one of those misleading errors that is actually caused by
something totally unconnected with the Device Converter ?

| PAGE COORDINATION LOST means the page numbering has gotten screwed up. In this
case it was reset to 1. Note 260 has some things that can cause this also an
included file with a title tag will reset the page to 1.
