I get the following error at all my HEAD1 and SECTION tags when
I use the online.reference bookreader option.

I am using DOCUMENT V1.2-A. I have successfully built this book
in the bookreader format with V1.0.

You are running a pre 1.2A version of the online tools. You need to
upgrade.
DOCUMENT V1.2B recently released and has the online tools bundled
with them: see note 270 for more information.
If upgrading to V1.2B is not feasible for you at the moment, you
can install a version of the online tools that runs under 1.2A.
See note 64.10 (and .11 and .12).
Having released V1.2B, we are no longer supporting earlier versions of
the online tools, so you are pretty much on your own if you chose
to install the saveset referenced in note 65.10. Our recommendation
is that you upgrade to DOCUMENT V1.2B for the latest versions of
both DOCUMENT and the online tools.
