See the DOCUMENT notes file, number 2163, for a bug in DOCUMENT that causes
index problems. The problems, with code example (and other?) fonts in index
entries, appear in both Bookreader and printed output, and didn't occur with
DOCUMENT V1.1.
If you experience this problem, you'll get error messages but the book will
process, and the index entries will be garbled in some cases. I got the problem
when I used inline <code_example> more than once in a single index entry.
