    The next two replies contain the SES performance review template, divided 
    into two parts.  The first part is the employee performance review 
    template. The second part is the manager section of the review.  It 
    contains a performance summary, rating and signature page.

Gather input from at least six colleagues and clients, and include those Mail
messages inside the <tag>(CODE_EXAMPLE) tags in this section.  The .SDML file 
of this template includes a sample message you can use to request review 
input.  You should try to get a broad sampling of reviewers; for example, from
your funder(s), peers from your information development team and from the
client organization, project leader or manager, or client information manager. 

You have the choice of deciding whether reviewers should send their input
directly to you, directly to me, or to both of us.  If input is sent to me, I
will share it with you so you can factor it into your evaluation.

Not every reviewer will give input in every performance area, but the total
input you receive should assess your skills and knowledge and give specific 
examples of problems you solved or special assistance you gave. Reviewers
should also provide feedback for areas where you can develop further.

Instructions:

1. Fill out the employee name, badge, etc. in the symbol definition section
2. Add Performance summary and rating
3. Insert employee section right before the signature page
4. Move the (Review Input) section to the end of the document, after
   the signatures.  I think this works better so they're really an
   attachment to the review.

Search on the COMMENT tag to read instruction for each section of the 
review.
