I'll mail you a copy of the form I've used for the past nn months. I have used
it as recently as a few weeks ago so I guess it still works. As far as routing
the request via the CC Manager's account for approval, it is also acceptable
for the CC Manager to write a "proxy" memo which YOU can attach to orders you
submit.

| Just type VTX IEG and you can have the current ordering forms and
other info mailed directly to your account. CC Mgr approval needed
as with all CC orders--see VTX IEG for details.

I know that all orders require CC approval. My concern was with my manager
having to email the order him/herself. .1 answered the question, and I have
the form now.

| Re .2, don't confuse IEG with DECdirect. Each have their own forms and
procedures. The base note author requested DECdirect data and I have provided
him with that off-line. However, as you indicated, the IEG forms are ALSO
available online via VTX IEG.
