Has a moderator of this conference (or any other conference, for that
matter, if you know) ever been asked in an 'official' way to reveal the
identity of an anonymous noter? If so, what steps did you take to comply
(or refuse compliance) with the request?

    No also - I've been posting anonymous notes for seven years now and
    have never had such a request.  But it's partly because I refuse
    to post notes which I think are likely to cause someone to
    "officially" ask for the name.
    
    I do point out to people that moderators have no "shield law" and
    that if *MY* management insists that I divulge a name, I will do
    so.  But otherwise, I keep it to myself.  (Indeed, I store the
    names in an encrypted file to which only I know the key.)  I
    don't even tell my wife, nor the other moderators.
    
    I've probably posted over a hundred anonymous notes, and the worst
    that's ever happened is that someone asked me to delete one
    (not that I'd do that either without the same proper cause that
    I'd want for any other note.)
    
    Anonymous notes are a useful, even vital part of noting.  It is
    important to do it in a way so as to preserve the corporate
    requirement for personal responsibility for what one has written,
    which is why I ask that at least one moderator be able to
    identify (or reasonably believe they can identify) the author of
    each note in a conference.
