The ZKO Health Services Department is conducting a survey of employees to
get their inputs into the design of a "Wellness Program" for the site.

The survey is on-line and everyone is welcome to participate (although the
program is being designed for Spit Brook residents, any employee input to
the survey is welcome).

Simply "SET HOST UCOUNT" (or 2283 if UCOUNT isn't defined on your system)
and log into WELLNESS, password WELLNESS. You will be asked a series of
questions and your responses will be anonymously forwarded to ZKO Health
Services at the end. The whole process takes less than 5 minutes.
