The following is an extract of a DECmcc-BMS installation log for BMS V1.3.
I pulled the kit from TOOK::USER$219:[OCONNOR.BMS].  I tried the installation
as an upgrade first and received the following message.  Then I deleted all 
DECmcc files from the system and tried the installation again but received
the same error.  Each time, I had removed all MCC installed images.

Does anyone have any ideas or suggestions?

Hmm..  My bet is that you don't have motif installed on your system.

    Try the following:
    
    	mcc> enroll mcc_iconic_map_pm
        mcc> show mcc 0 iconicmap 
    
    My suspicion is that these commands are failing.    
    
    Another question is do you see the ident mismatch error when
    trying to run the iconic map (mana/e/i=d)?
    
    If so, make sure your system has Motif V1.1.3 installed.

    It looks like I am running Motif V1.0.  I will upgrade and try the
    installation again.  I will post the results here.

    After upgrading to Motif V1.1 (installed off the Jan CDrom), the
    installation went smoothly!
    
    Thanks for the help / direction... I though I was already at Motif V1.1
    but apparently not.
