The file is PPINC::MCC_PUBLIC:[MISC]MAP_FORMAT.PS
(This is the location that is specified in 3.140. I think
you were missing the [MISC] part...)
Note that the document is not up to date. We are planning
to update it to include V1.3 information and will post a
note in this conference when that is done.
