DECmcc requires the use of a namespace service for the storage and
retrieval of network entity information. You can select a local
namespace or a distributed namespace (DNS). Select the local namespace,
unless you have multiple DECmcc systems that need to share namespace
information, or you are using a DECnet/OSI distributed namespace.
If either of those conditions apply, select the DNS; otherwise,
select the local namespace. You may choose only one namespace.
If you select the DNS, you will need to plan and configure the namespace
properly, before registering any entities in it. See the DNS chapter
in the DECmcc Planning and Installation for ULTRIX manual.
If you intend to use the DECmcc Autoconfiguration applications with
the local namespace, please see the Autoconfiguration and Autotopology
Use manual on how to plan your entity name structure.

Hmm. This is a tough one (and not helped by some 100-year old code
which simply returns MCC_S_KERNININTF in the face of any one of a
hundred possible errors).
Your trace shows the code that is failure has something to do with
creating the mcc_dispatch_table.dat file, or, is the first use
of the mcc lock manager, which involves creating and locking a
bunch of Ultrix semapahores.
These are areas to check out. Did you make the changes to /sys/h/sem.h
and rebuild the Ultrix kernel? Can you create mcc_dispatch_table.dat
in /usr/mcc/mcc_system out size the context of MCC? (For example
try a cat > /usr/mcc/mcc_system/mcc_dispatch_table.dat) and see what
happens (do an ls -l afterward to see if you created it).
Also, could you post what other software has been installed on this
system ?

Jim,
The sem.h file was adapted and we did a rebuild of the kernel. The cat
command just like you mentioned works fine.
Here follows the list of the software that we installed.
On one machine we use DECNET/OSI and the other DECNET,but the problem
is in both cases the same.

After a couple of painful hours of trans-oceanic dbx, this problem
appears to be a missing license.
There is a mis-coordination between the PM startup and the kernel
startup which causes the latter to notice the license problem first.
Unfortunately it is only the PM that is equipped to issue an
intelligent message.
This is in common code so I'm not sure it's Ultrix-specific. Could
happen on VMS too.
