    Why is the SAMPLE AM not any more an icon offered in the tool box, I know
    it was previous versions?
    
    The AM is enrolled correctly - I can click on it. There is also an 
    MCC_SAMPLE_ICON.DAT file in mcc_system.

The only reason it was there in the first place was 
because 

        1. the code that looked at the dictionary was
           not robust enough to skip over it.

        2. debugging purposes.

Last October the Iconic Map was modified to filter out the SAMPLE global
entity.  At that time it was considered buggy and caused the Iconic Map
to crash.   The short term fix was to not let the user select it until
the AM was fixed.  At that time it was unclear whether it was going to be 
fixed in time for v1.1.  If memory serves me correct (which might not be the
case here :-) all hell broke loose when a REGISTER SAMPLE xxx  directive 
was issued.   

I will check into whether SAMPLE should go back into the toolbox for v1.2.

    You can't register SAMPLE entities because they don't support a
    FULLNAME as one of their identifiers.
    
    We have had requests that sample be made registerable as an example of
    how to do it, as well as to have a fast-quick registerable "real"
    entity to test out domains/maps etc.
    
    I believe this will be done for 1.2 or some such time.
