I am attempting to install DECmcc/EXEC onto a VS3100 M38 running VMS T5.4-4GE.
Here are the final few lines of the installation procedure.

   VMSINSTAL asks no additional questions.

%VMSINSTAL-I-RESTORE, Restoring product save set B ...
%VMSINSTAL-W-NOFILE, New file MCC$STARTUP.COM  does not exist.
%VMSINSTAL-E-INSFAIL, The installation of MCC$EXEC V1.1 has failed.

The system disk is being loaded with layered products for the first time.  Only
a few (CMS, MMS, SCA, LSE, CMA) had been installed prior to the attempted
installation of MCC.  Hint:  I don't believe any of these use the SYSMAN startup
database.

Is this a bug or (hopefully) just a dumb user error?

    In note 171 you will find a pointer, from Steve Wong, to a kit that
    will be available on monday for VMS 5.4. The kit that you are using is
    quite old. We no longer have a DECmcc/EXEC we have a director kit
    and BMS (Basic Management System) kit.
    
    There is also an announcment note at the begining of this conference.

I saw that note as well as several notes at the beginning of the conference.
I wasn't sure whether the new kit would also correct this problem.
