I've seen the error show up in two different scenarios: 
 
1. I'm developing, compiling, linking, debugging.  Along the way, the .OLB 
become corrupted so that I can no longer insert object files into it.  
 
2. Our kitting process makes a copy of all of the files that go onto the kit.  
One of the files on the kit is an .OLB file.  Sometimes the .OLB file that is 
copied become corrupted in the destination directory, though now LIBRARIAN 
operation have been performed on it even.   The last time this occurred, I did 
a binary difference on the corrupted file and the source file.  The first two 
blocks of the corrupted file were zero-ed out.  There were also some later 
blocks that had been zapped.

Sharon,

I haven't found anything that points at the copy command or OLB files.
Can you give me some info of how the error exhibits itself?

Here is one thing I did find, although I don't believe it's related.

SYMPTOM:

Inserting an object module that has compilation errors into an object
library can corrupt the library, causing errors such as:

    %LIBRAR-W-LIBMODCNFLCT, module 'module name' is not Open
or
    %LIBRAR-F-BRKNLIB, library 'library-name' not properly closed -
     compress or rebuild
    -LBR-W-ERRCLOSE, Error occurred during a previous write to library
or
    %LIBRAR-W-HISTERR, error accessing library update history in 'library name'
    -LBR-E-INTRNLERR, LBR internal error. Please submit SPR
