Upon further investigation we found that the source TLB file (STARLETSD.TLB) was not in the expected TEXT format. Also, the errors seem to be indicating problems with SDL access to the STARLETSD.TLB file when it is trying to build the library for BASIC. We have checked everything and are now standing around scratching our heads. Please help.

The contents of SYS$LIBRARY:STARLETSD.TLB is not text - it is precompiled SDL structure definitions. Since you did not say what errors you were getting, nor what version of DEC BASIC you were installing, it's difficult to speculate further.
