This is the QAR I've just entered in the DW-IFT database, QAR 4125:

"In using a two head system, I'm find that 1-20 times per day the pointer
warps from one screen to the other.  Usually the pointer is already in
motion, so it's not always clear why the pointer disappears, but it does
`vanish' to the other screen.  I thought that this had something to do
with my pointer acceleration or something, but I discussed it with a
coworker who also has noticed the problem, with a similar frequency.

"My best guess at what is happening is that the pointer warps to the same
position on the other screen.  Since it isn't reproducible, I'm not
positive, but I think that it is just as likely to warp from screen 0 to 1
as from 1 to 0.  There is usually other activity taking place, like a
window being popped up of de-iconified or a text window scrolling."

Have others noticed this problem?  If so, can you describe what happens in
better detail?

If either answer is yes, please respond, or particularly if you can describe
a method of reproducing it, please QAR it (refer to QAR 4125 so they can
easily find the duplicate).

Well, Mulp, I can't describe it any better than you, but now that you mention
it...  It does seem like something weird happens to the pointer now and then.
I always just thought it was me loosing track, since I don't have the two 
monitors up tight to each other.

I'll try to pay better attention...

The one thing I can think of that I have seen is that if you have a 100dpi color
monitor and a 75dpi black and white, when you go off the right edge of the
color, you end up a few hundred pixels into the b&w rather than right on the
left edge.  This hysteresis can be confusing because a very small mouse motion
to the right can require a large motion to the left to restore it.

This does not exactly describe your symptoms, but...
