which operating system, which version(s), were you running any clients, if so

| I am the only one on the system, no clients. It is a VAXstation II/GPX
running VMS 5.2 and Decwindows V1. I boot off of another
Vax in the cluster. It sounds to be that a file is corrupt but I
cannot find out what file.

I am getting the error message:
%RBLD-E-ACCINDEXF, failed to access index file on relative volume 1
I cannot find this error message in any of my documentation nor
in any of the .MSG files on my system.
Can some one tell me where I can find info on this error message?

It is coming from a MOUNT without /NOREBUILD or a SET VOLUME /REBUILD. Mount
the disk /NOREBUILD and take a look at [000000]INDEXF.SYS. This error should
not happen in normal use -- something is seriously wrong with the volume.

Actually, this error has the potential for occuring everytime
more than 1 process performs a MOUNT /REBUILD or a SET VOLUME /REBUILD
at the same time on the same disk (i.e. MOUNT/CLUSTER/REBUILD).
One process performing the rebuild operation will lock out other
processes from doing a rebuild. ACCINDEX and ACCBITMAP are common
(albeit undocumented) errors.
