I've been able to re-create this under various V2 flavors of DECwindows:

Pause Session

Type correct password

Hit <Return> *twice* rapidly

Session resumes, then an "Incorrect Password" window appears... It goes away
once it's acknowleged, but I doubt that it's supposed to appear in the first
place.

I couldn't do this with SSB DECwindows.
