    The Post Office near the McDonald's carries some Massachusetts tax
    forms.  I don't know if they have extension forms.

Now the rest of the story...
  It's in Adobe Acrobat format so you need an Acrobat reader to get to it.
  Unlike some other forms out there, this one isn't in the proper color of ink
so the one you get off the Web has something like "Sample copy only - Can Not Be
Filed" stamped in giant letters across the front. You can thus use it for
telephone filing and little else.
  If you filed your previous year's taxes and don't owe anything to the state on
your 1996 income, you can file it via pushbutton telephone - it records you
giving your name and Social Security number after you've punched in all the
other data it wants.

  If you need to send in the form, you could try the Tyngsboro post office which
if I recall is just a couple miles down the road from Pheasant Lane. (call first
to see if they have it).

    Try the Massachusetts Department of Revenue home page.  I did find the
    form that you needed, but it is in PDF form.  There are instructions on
    the form indicating that it should be done by telephone.  There are
    also conditions which you must meet to do this.

    Thankx for all the answer(s). I had already gone down the web path, but
    as you noticed, the form is "for informational purposes only". Since I
    don't fir the criteria for doing by phone - that one was out. I finally
    picked one up in the Newtonville PO near my house.
    However, the strangest answer was the one that I got from calling the
    DOR:
    Take the Federal one, cross out all mention of Federal and replace with
    "Massachusetts".....
