804.1V1.2 is latest microcode for DEFZATKTVFS::IDONaoki Ido, CSC/TOKYO, EWB, DTN 680-2456Wed Dec 02 1992 02:347
The latest DEFZA firware is V1.2. You need DEFMA micro code (DEFZA012.SYS)
and DECndu utility software in your VAX4000 for loading the code into DEFZA
board. You should order the FDDI upgarde kit or contact to local CSC to get
both images.

Note that NDU support for the DEFZA is not in the VMS V5.5-2 DEFZA
driver (FCDRIVER). This support is scheduled for a future release.

In the meantime, you'll need to update your controller on an Ultrix
    DECstation (using NDU on Ultrix).

Good luck finding a network copy of the firmware. I'm not aware of a
    good copy (anymore).

You must have an old/prototype board for it not to be V1.2 already.
If your board is hardware rev C01 or before (look at the bulkhead),
you might have trouble when you use the device.

re.3
The DEFZA came from an ULTRIX workstation originally.
Does this mean we need to install the DEFZA back to the ULTRIX WS, run
NDU and put it back to the VS4000?
thanks the info

I checked whether ndu-vms works for DEFZA under VMS V5.5-2. It didn't work
for DEFZA as the following. So, you have to upgrade your DEFZA in ULTRIX and
then move it to VAX4000-90.
