    "Options->Customize" does not reset if you click on [Cancel] and
    re-enter the window.
    
    Go in & customize, delete/rename some buttons on your toolbar, then click
    [Cancel].  Re-enter the customization window, and the toolbar is how
    you left it when you clicked [Cancel].
