A complete solution is below. Here's a hint: You can block all the
monsters in the bottom half of the room, and you can avoid the
fire-breathing monsters until the end.
Get all the hearts outside the lava-enclosed areas.
Push the right block right so you can get above it. Then push it down
one space, then left half a space, and then down a space. The block in
this position blocks monsters from below and blocks the skull on the
right from moving left.
Push the bottom left block right until you can get above it. Then push
it down. In this position, it completes the blocking of the skull
below it.
Make a ladder in the lava just above the upper left block. Get the
five hearts in that lava-enclosed area and then push the block down.
Then push it left, leaving it in a half-way position across two spaces,
blocking the monsters from below.
Make a ladder and get the two hearts inside the other lava-enclosed
area. The monsters will start to move, but they can't bother you, so
take your time.
Go around toward the chest. From a short distance, shoot twice at the
monster below the chest.
Get the chest and leave the room.
