| Rich, do you know what they've tried so far?
It could be a couple of things since it appears to be so isolated.
One thing would be the spacer on either side of that cog. If they
haven't tried already, I would recommend a different spacer. BTW,
Shimano spacers can work (so you don't sink any real money into it).
Or it could be the cog. I doubt if it's dish or chain alignment.

| I can think of some possibilities.
First, is the rear derailleur running as close to the block as it can be?
The closer the upper jockey wheel is to the block, the more positive is the
shifting. There's an adjustment screw for this, right where the derailleur
screws onto the frame. If you screw this screw most of the way out, it allows
the derailleur to 'contract', bringing the upper jockey wheel closer to the
block. The limit to this adjustment is when, if you back-pedal, that jockey
wheel starts to touch the block.
Secondly, are the cassette sprockets assembled in the correct relationship
to each other? My Campag cassette came with a very informative leaflet that
included a chart showing the rotational relationship for every permissible
adjacent pair of sprockets.
Finally, if all this fails, I understand that Campag have just released a
new type of cassette sprocket that is, essentially, hyperglide - they have
those little steps machined into their sides. These new sprockets could cure
your Campag shifting blues.

| Rod brings up some other good points. The new sprocket arrangement
is called ExaDrive. I haven't heard or read anything on it yet, just
advertisements.
The adjustment barrel on the cable guide is just for some very fine
tuning. I've never found them to be effective for serious adjustments.
There should be no reason to have clackity gears. And all should shift
crisply and stay that way fairly consistently (with the exception of
new cable stretch).
Most of my gears a dead quiet. I only notice real noise when I've
neglected my duties (chain lube).
One other thing just struck me... I have to squirt a little oil in
the cable housing at the derailleur section, BB shell cable guides,
and at the down tube cable guides. They do get dirty from time to
time and I do notice shift degradation. I always make sure the cables
are really lube'd too. Regardless of the teflon lining...
